The Adult Congenital Heart Disease Patient and Family Advisory Council (ACHD PFAC) was formed to acknowledge that patients and families are an essential resource to UC San Diego Health.
This group is made up of patients, providers and staff who are committed to ensuring that your opinions and perspectives are heard. ACHD patients or family members and caregivers are welcome to apply to join the council.
Partnering with patients and families will enhance our processes, procedures and delivery of care.
Purpose and Goals
The Adult Congenital Heart Disease Patient and Family Council aims to create a culture where everyone feels acknowledged, respected, and valued throughout their health care journey.
Identify opportunities
to improve processes and program development that will benefit patients and family members.
Improve the patient experience
by focusing on how we can provide excellent patient care, help patients navigate the health care system and increase awareness of services and support available in the community.
Promote better communication
to foster relationships so patients and their family members feel heard, respected and cared for.
